Now you can receive timely and cost effective second opinions from David
Melton & Associates. Here's how it works:

Fill out and e-mail or print out and fax to (970) 925-7618 the Second
Opinion Mail Back Form below.

E-mail, fax, or express mail the expert's report or reports to us at David
Melton & Associates, 135 W. Main St., Suite A, Aspen, CO 81611.

Within five business days of the receipt of the expert's report (or sooner
in emergency situations), we will e-mail or express mail to you, our review
and analysis of the report and set up a telephone consultation with you
to review our findings.

Upon receipt of your completed "Second Opinion Mailback Form", Mr. Melton
will call you to discuss our service and the cost to you. This call of
up to 15 minutes will be at no charge to you (For attorneys and CPA expert
witnesses only). No charge to your credit card will be made until we have
performed our analysis of the "Expert's Report" and e-mailed or express
mailed our analysis to you. After receiving the "Expert's Report" and prior
to beginning our analysis, we will pre-authorize your credit card for the
agreed upon amount of our service.
